** MUST HAVE ALL forms to be assisted. Thank You!
- Drivers License or Picture ID of all adults in household
- Social Security CARDS of all in household (adults & children)
- Proof of income/money received in last 30 days
- Proof of residency (correct name & address)
- Proof showing why in crisis - any receipts, statement, etc.
